Abstract
서 론
congenital syphilis는 오랜기간 질병을 박멸하려는 노력에도 불구하고 개개인의 무지와
성교육 및 성병교육의 소홀과 관심미흡, 정부나 사회전체적인 무관심 때문에 해마다 점점
증가하는 추세에 있다
congenital syphilis는 임신 16주경 초기 placenta의 Langhan's cell layer가 퇴축되면서 모
체로부터 fetus로 감염되는 intrauterine infectious diseases의 하나로 mother's german
measles (rubella)등과 같이 태아의 경조직 및 연조직과 전신장기에 많은 장애를 유발하며
심한 경우 태아의 사산을 유발하기도 한다.
오늘날 congenital syphilis는 예방이 가능한 질병으로 감염된 모체를 blood test 등을 통
하여 빨리 감지하고 임신조기에 적절한 조치를 해 줌으로써 퇴치할 수 있다.
congenital syphilis의 증상은 primary또는 chancre stage가 없는 것을 제외하고는
acquired syphilis와 거의 유사하지만 감염시기 및 모체의 감염도에 따라 상당히 다양하게
나타나며 이러한 증상과 다양성 때문에 congenital syphilis의 정확한 임상적 진단에 어려움
이 있다.
그러나, 여러가지 증상 중에서 구강내 증상, 특히 Hutchinson s teeth와 Mulberry molar
등은 late congenital syphilis의 특징적인 중상이며 진단에 단서가 되는 것으로 치과의사는
이에 대해 잘 알고 있어야 하며 이와 연관하여 다른 전신적 문제점을 잘 평가할 수 있어야
한다.
저자는 서울대병원 소아치과에 내원한 11세 남아에서 congenital syphilis의 소견을 관찰
하였기에 이를 보고하는 바이다.

Today, Ccongenital syphilis is a preventable disease. ongenital syphilis usually results
from transplacental infection of the developing fetus, and hematogeneous infection can
occur throughout pregnancy. Manifestations of late congenital syphilis include concave
profile, saddle nose, protuberant mandible and hypoplastic tooth, (Hutchinson's tooth,
mulberry molar). Most of the stigmas described in the literature may occur in other
conditions and are etiologically indistinguishable. Only Clinically characteristic
Hutchinson's tooth, mulberry molars and interstitial keratitis amy be classed as
pathognomic. A case of congenital syphilis in a with oral manifiestation is precented.
